Was delicate and constantly ailing in childhood. 12 years ago fell from a cart and, as he thinks, consequently up to 3 yrs [years] ago passed from time to time blood intimately mixed with his urine. Has often experienced pain across his loins. Since childhood has been subject to pain on left side between lower ribs and hip. Pain relieved by resting on that side. For 2 or 3 months past has resided in Paris. 8 days ago caught cold in a Railway station. 4 nights ago had an attack of shivers, repeated constantly since. Has felt very giddy lately. Has been taking purgatives character of [?] variable. Complains also of headache and pain between shoulders'
