He had been in bed at home for 5 weeks. Had lost flesh. Jan [January] 1896 he was ill for 3 weeks with sciatica in R. [Right] thigh and 26 years ago was in Middlesex hospital with rheumatics and erysipelas of left elbow. A sister died of consumption 20 years ago. Alcohol moderate. Syphilis 25 years ago'
