Medical notes: 'Case not seen by Reg [Registrar]: H.S. [House surgeon] reports. Patient had been ill some 10 days: he was lifting a weight and thought that his rupture had come down. He was constipated for 4 days, then there was some action of B. [Bowels]. Since then (6 days) constipation absolute in spite of copious enemata. On admission patient was collapsed with thready pulse, hiccough and faecal vomiting present'
