Medical notes: 'History: She had been ill since an influenza like attack which was followed by rheumatic fever 6 months ago. A week ago she became paralysed down the lt. [left] side. On admission: 8/12/'21. Anaemic and wasted. T.P.R. [Temperature, pulse, respiration rate] 97.8, 104, 20. The lt. [left] arm, leg and face were paralysed and flaccid. There was a Babinski but absence of abdominal reflexes on lt. [left] si [side]. Lungs. A few coarse rhonchi. Heart. Not enlarged. Systolic and diastolic mitral murmurs. Aortic sounds pure. No petechiae. A trace of albumen in the urine. Course… A streptococcus (of the viridans type) was isolated from the blood. She was treated with an autogenous vaccine. Some degree of recovery from the hemiplegia occurred. However there was pyrexia throughout and about a month after admission she had a rigor and 5 days later she died. 14/1/'22'
