Medical notes: 'When young the patient had acute rheumatism. Four and a half years before he came to St George’s Hospital he had a second attack: he was treated in Birmingham Hospital. Twelve weeks before his admission he had rheumatic fever for the third time: he was in bed for about 6 weeks. He then recovered. Three weeks before he came to the hospital he said that on lifting a heavy beam he had sudden, sharp pain in the region of the heart’
