History Present Illness – Loss of flesh for 4 months. Swelling of the legs for 6 weeks. Dyspnoea on and off for 4 months. No work for 1 month, in bed for 3 weeks. He complained of dyspnoea and pain in the stomach and of shortness of breath. The pain was a constant [?] one, worse on standing up'

Tumour in the abdomen had been noticed about eight months. She had been getting progressively weaker since the beginning of her illness but particularly so recently. Shortness of breath had been present a month and had been getting worse. Bowels had recently been regular. Nothing in the history. The menopause had occurred seven years before admission without any trouble'
