Onset somewhat gradual except for a slight cough of 1 mo's [month's] standing. 1st to 6th days: - frontal headache: Rt. [Right] earache: anorexia: night sweats: dark urine: confined bowels. Cervical abscess at 3 or 4. Lively disposition: not remarkably clever. Loss of flesh for 6 mo [months]. ? Injury to spine. Nine[?] days before admn [admission] he came home, looking very pale and saying that a workman had seized him and shaken him by the hair. Medical notes: 'History. Mother d. [died] of consumption: previous history of good health. Quite well till noon on the 7th day before admission: when she was noticed to have a troublesome cough. Towards evening she seemed to be struggling to spit something up and during the night there was severe dyspnoea with marked stridor. Next day all the symptoms were somewhat relieved. During the night they again became severe. On the 3rd day all symptoms had disappeared except a slight cough: she ate with a good appetite. This state of things continued till the 6th. On the 6th she complained of sore throat. The night before admn [admission] the severe dyspnoea and stridor returned, continued all night and part of the next morning, then lessened: as she was being brought up in a cab, these were hardly noticeable, but seemed to return on admission. No nasal discharge or swelling in the neck noticed'

Medical notes: 'History. Had suffered from enteric, scarlet, and rheumatic fevers. Appetite moderate: lived chiefly on soup and vegetables: ate little meat, very little butter, and no fat: occasional diarrhoea in summer: diaphoresis habitually profuse: possibly from the nature of his work only. He drank a bottle of claret a day: no beer or spirits. Coughs in winter since coming to England )15 years). He went to France on Aug [August] 22nd 1878, being then apparently quite well. Next day, as he was lifting something he brought up about 3 pints of bright blood, without effort either of coughing or vomiting. After this, except that he felt very weak and lost flesh, he remained without symptoms till some six weeks before admission, when cough gradually commenced, without rigor or sign of coryza. It steadily increased, with dyspnoea, but hardly any expectoration. His appetite gradually failed, he continued to lose flesh. His bowels acted loosely twice or so a day: five or six times the last day or two before admission. No rigors or night sweats: no more haemoptysis'
