Medical notes: 'History. Illness started 2 years ago with epigastric discomfort. He passed blood with his stools a year ago and again a fortnight ago. About a year ago he had cough and occasional haemoptysis. Since then he has had bronchitis and shortness of breath. On admission. 23/11/21. T.P.R. [Temperature, pulse, respiration rate] 98, 150, 21. Pt. [Patient] was very weak and had difficulty in speaking. Chest. No movement upper part of rt. [right] side. Whole rt. [right] lung dull. Signs of cavitation rt. [right] upper lobe'
