After this, he had pain at the end of the penis after micturition, the stream of urine would sometimes suddenly cease, & then on altering his position flow again, it was also occasionally tinged with blood. Medical notes: 'History. No heritable disease in the family. Rheumatic fever 15 yrs [years] before adm [admission]: scarlatina in childhood. Health otherwise good. Averaged 5 pts [pints] of beer and 1 glass of gin daily. Thirty three days before admn [admission] he got wet (not an unusual thing). In the evening pain appeared in the loins, which continued. On the third day the face was found to puffy, and the urine dark. On 4th or 5th the lumbar pain receded and the legs swelled: face became less puffy. About the 6th the abdomen swelled and became somewhat painful: He laid up. The swelling of the legs slowly increased; the urine became lighter: the bowels were very costive. Slight cough and loss of appetite for 3 days before admission'
