Medical notes: 'History. Thirty days before admission: - a rigor for two hours; at the end of which time he found his throat sore and the submaxillary region swelled. The soreness and the swelling increased for five days; the latter being considerable, and extending all round the front under the chin. There was no nasal discharge. He was treated for mumps. For a week more the condition remained stationary: then the soreness and swelling began to recede. From that time he was unable to swallow his food properly, as the ingested had a tendency to regurgitate per nares. He took solid food, but it seemed to 'lodge' causing substernal pain, and was generally vomited within half an hour; liquid being retained'.

About 10 yrs [years] before admission, he was laid up 13 weeks during the latter part of the summer with debility and pains in the R. [Right] side and elsewhere, which his medical attendant called 'dyspepsia': During the early part of this time he found that his perspiration had altogether ceased. About a month after the cessation, he commenced gradually to cough: since then he had been free from cough only in the hot weather. It usually attacked him in paroxysms of half and hour's duration, in the morning after going out of doors and in the cool of the evening. He had also suffered from dyspnoea, which was severe in hot weather, and from frequent attacks of chilliness and shivering, except in hot weather. For 3 yrs [years] the paroxysms of cough had ended in free expectoration. During the same period he had suffered from palpitation. He lost flesh from Xmas 1877'

His wife stated that he was subject to severe and prolonged coughs, to sick headaches, epistaxis, and boils: he habitually sweated profusely, was intolerant of fat, and drank much more than was good for him. At Xmas 1876 he was laid up a month with R. {Right] pleurisy: he got quite rid of the symptoms from that time till the following May: then was laid up 3 wks [weeks] with inflammation of the R. [Right] lung. From that time he was never free from cough. In Septr [September] 1877 he suddenly coughed ' a pint' of dark blood: continued to bring up more or less for a fortnight. From that time there was much expectorn [expectoration]. Four similar attacks of haemoptysis succeeded, the last at the end of April 1878. The cough steadily increased: he sweated profusely. He lost flesh from the middle of April. A rigor for three hours, 12 days before admission'
