Medical notes: The patient was a carman of fairly temperate habits whose first illness was gout in 1880. He had had a winter cough for the last four winters, but had never been laid up until October 1885 when he had bronchitis. He had bronchitis on and off ever since and oedema of the legs came on three weeks before admission.
