The patient, a warehouseman or store-keeper, had enjoyed very fair health, except that in 1854 he coughed up a little blood without being seriously ill. It did not appear from the history that he had been addicted to the abuse of alcohol. Eleven weeks before admission he began to lose strength and appetite, and noticed enlarged glands on either side of his neck. Six weeks later he vomited a large quantity of blood on two successive nights, after which he kept his bed. About a week before admission he had several attacks of violent shivering'

He was formerly in the navy but had no foreign fevers. In 1867 he contracted syphilis with secondary symptoms. His health was good till October 1883 when he was operated upon by Mr Holmes [Timothy Holmes] for fistula in ano. In April 1887 he was under Mr Rouse's [James Rouse] care in this hospital, and a large abscess was opened which had existed for some months over the left lower ribs. This abscess was not an empyema'

For 4 or 5 months he had been liable to morning sickness. For 5 or 6 weeks the abdomen had been swelling, and there had been free diarrhoea'

There was a clear history of the onset of illness exactly three weeks before his admission. His account was that in the course of a month's stay at Scarborough he ‘caught a chill' one day by exposure to a cold wind while in a sweat. After this he felt out of sorts with ‘general debility, loss of appetite and rumbling in the ears'. He also had irrefular shivers and sweats'

In the autumn of 1884 he was in this hospital for a month under Dr Dickinson [William Howship Dickinson], with oedema of the legs, enlargement of the liver and slightly albuminous urine. He improved greatly under treatment and modified his habits somewhat in the direction of temperance. About 1887-8 there was great oedema of the legs for a time, and then he became almost a total abstainer from alcohol'
