Took 4 pts [pints] of beer a day, with 'a great deal' of spirits. Venereal sore 16 yrs [years] ago: no bubo or constutl. [constitutional] symptoms. Cough with occasional exacerbations since the age of 3. Early in Dec. [December] he had pecuniary troubles; became weak and languid: lost his appetite. Severe cough then for 10 days. Occasional streak of blood spat since Xmas. About the 2nd week in Jany [January], laid up with severe catching pain in R. [Right] flank, with no increase of cough; which lasted altogether six weeks. Lost flesh from that time. Increased cough, 2 mo [months]. Slight pain in flank 1 wk [week]'
