Medical notes: 'Family history nothing of importance. No previous chorea, no scarlet fever, Rheumatic fever twice. 1st when 7 years old, 2nd 5 years ago. Present illness began 3 weeks before admission with pain in back and head went to bed at once and remained there till admission. The choreic movements began 2 or 3 days before admission 'all over her' and gradually became worse'
