Medical notes: 'Father died of 'rheumatics, heart disease, and pleurisy'. Six years ago patient fell from a house and broke his wrist, thigh and jaw. I.P. [In-patient] here, no special note[?] as to abdomen. Quite well since then till present illness, Dec [December] 26th Had '2 or 3 little drops or Port wine' which made him sick, pains in belly, vomited two or three times every day since'
