She had been ailing for three years. The first symptoms she noticed was a difficulty in the act of defaecation, owing to the aperture of the anus, or the lower part of the rectum being narrower than in a state of health. She can give no account of the origin of her complaints, but the symptoms have become gradually more severe up to the present time’

She stated that she had noticed some degree of swelling for about six months at the upper part of the sternum’
