She was admitted on Aug [August] 21st with extensive oedema, cough, dyspnoea and cardiac pain. She was unable to lie down. There was a history of rheumatic fever 11 yrs [years] and again 7 yrs [years] before. The heart's area was much enlarged, the apex beat widely diffused: felt in the epigastrium: systolic murmur all over, also in the cervical vessels and in the back; culminating, loud and rasping, at the apex. […] She was readmitted on Dec [December] 19th suffering from intense dyspnoea, which prevented examination and interrogation'

Medical notes: 'History. One sister d. [died] of 'consumption of the bowels': another, and the mother also, of hemiplegia. Subject to sore throats: to palpitation for 3 yrs [years] before admn [admission]: diaphoresis habitually profuse. Catamenia regular, with dorsal pain till the beginning of 1878: from that time though still recurring duly, they were variable in amount, from scantiness to great profusion, and in duration from 1 to 10 days. Vomiting. For two years before admn [admission] she had suffered from vomiting of clear fluid twice or thrice a week; had suffered from epigastric and dorsal pain after eating; had lost flesh and strength. Rigors. For two months less she had had a rigor (10 min [minutes] durn [duration] ) at least every other day, generally daily about 1 or 2 p.m. after taking a small walk: following by heat and sweating. (This last occurred 2 days before admn [admission])'
