Slight nasal catarrh, sleeplessness, and deficient appetite for about 4 weeks. Complete anorexia supervened 15 days before admn [admission]. Next day, suddenly seized with severe dyspnoea: he almost fell down, and clung to the railings for support. In the evening he began to cough and spit. Coughing seemed to 'bring on' pain in the L. [Left] flank. The symptoms remained pretty constant thenceforth: he sweats a good deal and lost flesh: no rigors'

Medical notes: 'History. Mother died of mammary cancer. Nothing remarkable in constitution or previous history. In February 1878 she fell over a clothes prop[?] and struck the ground with the abdomen: much pain in the epigastrium and faintness ensued for the day. A fortnight or so later profuse vaginal haemorrhage commenced, which continued till about the middle of October. Except that she grew paler the general health was at first unaffected. From about August there was a 'fulness' of the abdomen after meals. Early in October the appetite failed. The haemorrhage having ceased, the abdomen began to swell, and the patient commenced to vomit after everything taken. Under treatment the vomiting ceased after a fortnight. The abdomen continued steadily to enlarge, the legs swelling slightly'
