This swelling increased in size and was noticed to pulsate first about Xmas last. He had been a policeman, and had attended his divisional surgeon for some time before his admission'

Shortly after it began he fell into an area: was insensible for 5 minutes and fractured his clavicle. While laid up with the fracture he had two[?] 'fits': nature unknown. Five other fits at one time 3 or 3 ½ yrs [years] later. Twenty eight days before admission he was struck in the epigastrium by the pole of a dray: the pain was severe: at St Thos' Hosp [St Thomas' Hospital] he was told he had fractured some lower R. [Right] ribs: strapping and bandage were applied, and retained till admn [admission] to St Geo [St George's Hospital]'

Medical notes: 'History. Attack of rheumatic pains 7 yrs [years] before admn [admission]: smallpox, scarlatina and typhus in early life. A healthy man, without constitutional peculiarities or discovered hereditary tendencies: accustomed to take 1-4 pts [pints] beer and 2 gl. [glasses] of whiskey daily. On Jan [January] 10th, attacked with shivering and general aching on coming out of a hot room. Cough commenced next morning, with clear expectoration (no nasal catarrh). It continued from that time, with frequent shivering, and cold sweats at night; the bowels being regular and the urine as usual. Three times it nearly disappeared under treatment, but relapsed as often, with a similar onset: - rig. [rigors] on Jan [January] 30th, Feb [February] 4th and Mar [March] 11th'
