At the age of 16 he contracted a chancre, not followed by secondary symptoms. Subsequently he earned his living as a commission-agent, and was for long a very heavy drinker of whiskey and beer. Upon the whole his health was fair except for an attack of gout about 1884 till some fistulae in ano began to form for which he was admitted into this hospital in May 1886 […] In October 1887 he was readmitted under Mr Haward [John Warrington Haward] complaining of pain in the right upper extremity, the veins of which and of the right side of the thorax were enlarged'
