Medical notes: 'For five years this patient had suffered from pain in the epigastrium and between the shoulders after food - She occasionally vomited after food. No history of haematemesis - A fortnight ago she was attacked with severe pain in abdomen and back and this was followed by swelling of the belly. The pain came on suddenly and was so severe as to make her faint - On admission she was found to be an emaciated woman breathing somewhat quickly and complaining of pain and swelling in the stomach'
