From that time this symptoms gradually increased, expectoration occurred and became somewhat copious in amount, while 'difficulty in breathing' after a fit of coughing was noticed about Xmas time. For a few weeks 'cracking' of the voice when speaking had been noticed: then suddenly the voice became very weak on Jan [January] 31st. He had served in the army. He was a married man with 6 children, alive and well'

Pneumonia twice. Occasionally in foggy weather cough and expectoration. A large eater: had got stout lately. Alcohol – beer, generally quite moderate, occasionally a drop too much. Otherwise previous history negative. Hist [History] of last illness – At work as usual on Sunday night Jan [January] 17th. Jan [January] 18th feeling queer: went to consult a doctor: no appetite for breakfast, took only a glass of rum and milk. At 2.30 p.m. suddenly fell down in the street, just outside the post-office, while carrying a bag of parcels. No struggling, immediately unconscious. A neighbouring woman took him in and tried to restore him on the floor of her parlour. There he remained unconscious for 24 hours. Finally a doctor advised his removal to this hospital'

History of diarrhoea for about a year and occasional morning sickness. No alcohol or syphilis. Speech slow, fingers clubbed. Spleen thought to be enlarged. No signs present in the lungs. Systolic murmur at the apex of heart and evidence of arterio-sclerosis. Liver not enlarged, rather diminished. Legs not oedematous. Urine normal'

On the next day there were well-marked signs of a left pneumothorax, During the next fortnight signs of ascites developed and the left chest seemed partially to be refilled with fluid'
