Medical notes: 'This woman, who had suffered for 3 or 4 years from cough and palpitation of heart, stated that she had not had any serious illness previously. She had borne 2 children, and the catamenia [menstruation] had ceased 10 or 12 years ago. Her cough had been very troublesome, and in a paroxysm she had ruptured herself about 12 months previous’
