Attacked with shivering about noon 12 days before admn [admission]: had to leave work and go home. Pain in L [Left] hypochondrium, increased at night, after food, or on inspiration; cough, dyspnoea, and occasional palpitation commenced, and continued. Shivering lasted two days. Bowels costive: urine as usual'

Medical notes: 'History. Father's brothers gouty: diaphoresis habitually profuse: fat always deprived him of appetite. Otherwise healthy. He took 2-3 pints of beer daily with occasional excesses. Slight nasal catarrh, sleeplessness, and deficient appetite for about 4 weeks. Complete anorexia supervened 15 days before admn [admission]. Next day, suddenly seized with severe dyspnoea: he almost fell down, and clung to the railings for support. In the evening he began to cough and spit. Coughing seemed to 'bring on' pain in the L. [Left] flank. The symptoms remained pretty constant thenceforth: he sweats a good deal and lost flesh: no rigors'
