Medical notes: The patient was a teetotaller. Seven years before he had suffered from severe lumbar pain and at that time he first noticed that his ankles swelled a little in the evenings. He had had polyuria for many years. For three weeks before admission there had been much oedema of the legs, cough, and dyspnoea on exertion.
