Medical notes: 'The patient’s mind was not clear, a history was obtained with difficulty and the man’s statements did not appear to be very reliable. Six days before he came in he said he had vomiting, headache and pain in his chest. He was however able to get about till the day of his admission. The man’s wife thought he had been injured, but there was no clear evidence of this, and there was no objective sign of it. The patient said he drank only a pint of beer a day, except when he ‘had company’ ‘
