Medical notes: 'Patient was admitted on April 5th for weakness and shortness of breath. A fortnight before admission he had had a 'bilious attack', his appetite had failed and he had felt sick. Ten days before admission he had had an attack of pain in the right supra-scapular region radiating to the neck, sufficiently severe to prevent sleep. Increasing shortness of breath for the 14 days preceding admission. There had been morning retching for years. Winter cough two years with expectoration and loss of weight. Previous history of rheumatic fever and syphilis. Nothing of importance in the family history'
