Medical notes: 'There was no history of syphilis obtainable - The chief symptom of this patient was that he had cough since May so severe that he had to give up work on account of it - For two months he had had slight difficulty of swallowing and for three weeks indefinite pains in the chest - much frothy expectoration and occasionally a sensation of choking - There had been no marked loss of flesh or change of voice - For two months he had had considerable dyspnoea on exertion and had had to be propped up in bed at night'
