Medical notes: 'This patient on her entrance to the hospital was in a state of great debility & presented a cachectic appearance, her complexion was pale & sallow. She had been ailing for three years. The first symptoms she noticed was a difficulty in the act of defaecation, owing to the aperture of the anus, or the lower part of the rectum being narrower than in a state of health. She can give no account of the origin of her complaints, but the symptoms have become gradually more severe up to the present time’
