Medical notes: 'This case, admitted on Jan 17th, was at first regarded as one of rheumatic fever. The history obtained, however, and the aspect of the man, from the first, suggested a different condition. He was low in bed, rather deaf and wandering, very weak, with a tremulous tongue, cleaning in patches, slight cough, a pulse of 120 and no swelling or tenderness of any joint'
