[Previous Medical History] Rheumatic fever aet 25 years. Osteoarthritis of hands and feet for the last 15 years, treated at Bath and Brixton. Often gets severe attacks of dyspnoea. Bowels rather constipated. Present Illness: Began 1 week before admission with shortness of breath, pain after food and general weakness. She has had to lie up for the past few days and has slept very little. She is unable to take her food at all well'
