Medical notes: 'The patient gave no history of rheumatism. He had bronchitis in January 1885 and was laid up in bed for a fortnight. Three weeks before his admission he had a sudden attack of syncope. There was no paralysis. For 10 days before he came in he felt very weak; for a week he had oedema of the legs’
